Request Details
- Please enter a short description to explain that nature of this request. Please include as much detailed information as possible in the detail field including any Workday reference ID's such as the JV reference number or other operational transaction reference ID's.

- This individual will be contacted if further information is required.
- This field defaults to the User logged into TeamDynamix, but you may select any active UNLV employee.

- This is the high level department that the Requestor belongs to.
- TeamDynamix will automatically populate this field based on who is selected as the Requestor
