Overview
SLT provides assistance and processing for Disposal, New, Replacements, and Lost cards:
- Card Disposal:
- Drop off card(s) at SMT front desk, label them with the word "dispose/disposal". Please indicate if this card/number will no longer be used, so we can remove access.
- New Workcards:
- Will need to be requested through the Online Access Request Form. You will need to specify the type of access needed as well as the template name.
- Replacement Cards:
- The Rebelcard office bills each department $10 for each workcard print/reprint, billing occurs monthly. Indicate which department this card is for and if this card has been lost.
- Report a lost or missing card:
- Indicate if a card needs to be deactivated or replaced. If you are not sure what the number on the card was, please provide as much information as possible to assist us in locating the number for said card.
It is imperative that lost cards be reported immediately to adhere to building safety/security standards for Student Life spaces and employees.

To ensure a prompt processing time, please provide a detailed description of your request, including all relevant information. The more specific and descriptive you are, the better we can understand and address your needs efficiently.
Request Card Support:
Having issues with your card? Please indicate the following:
- Card number?
- How long has this been happening?
- Did you get a replacement card recently?
- Which systems is your card having issues with (Onity, Salto, Blackboard)
Please provide as much specific information as possible to help us accurately identify and address the issue you are experiencing. The more details you provide, the better equipped we will be to assist you effectively.
