RMS Notification of On-Site Contractor

The following checkbox items and questions are to be answered if any contractor will be used in a project and will be coming on-site.  If any of the below items are going to take place on a project or work assignment, this document will be sent to the RMS Director with a cc: of all RMS Dept heads in order to help assist that proper safety protocols are in place. 

  • Aerial Lift (boom and scissor lifts)
  • Work Platforms (scaffolding)
  • Bloodborne Pathogens
  • Confined Space Entry 
  • Trenching (5ft or more)
  • Energized Electrical Work 
  • Fall Protection 
  • Powered Industrial Trucks (forklifts)
  • Hazard Communication
  • Hot Work (welding, cutting and brazing)
  • Control of Hazardous Energy
  • Occupational Noise (high level)
  • Respirable Crystalline Silica (grinding of concrete/tile)
  • Respiratory Protection 
  • Lockout/Tagout (equipment needing to be isolated)
  • Appropriate Certificates of Insurance from the Contractor and all subs (if applicable) received 20 days PRIOR to work beginning

Contractor Questions for RMS

Please ask the following questions of your contractor as part of the Planning Phase of the project. 

  1. What is the name of the UNLV project manager and phone number?
  2. Is there any contact information for the general contractor's, subs, and safety representatives?
  3. Will the work processes involve hot work, confined space entry or trenching? If so, can OSH obtain a copy of the contractor's procedures?
  4. Will the work processes involve the disturbance of asbestos containing materials or the generation of silica dust?
  5. What chemicals will be used on the job and can RMS get copies of the SDS for these products?
  6. Will there be UNLV employees and/or students in the areas where contractor work will be performed? If so, how will they be protected from contractor's activities?
  7. Will contractor operations interact with pedestrians and vehicles? If so, how will pedestrians and vehicles be protected?
  8. Will new vaults be installed as part of this project? What will the vaults contain and will there be an opportunity for RMS OSH to document by photographs?
  9. If work is being done on fire protection systems, they need to provide copies of state issued licenses on the employees and a copy of the state issued permit. 
  10. Will any fire protection systems, modification of existing systems, installation of a new system, replacement of outdated systems or elimination and removal of a system take place?  
  11. Will there be any modifications to walls or room designs that affect the sprinkler coverage or notification appliances (horn/strobes)? 
  12. Will the project add or replace boilers, chillers and generators?
  13. Will there be a change of Use of the building or space from originally approved?
  14. Will there be a placement of dumpsters near buildings?
  15. Will there be an Impairment of Fire Protection Systems? (may need fire watch and notify RMS Insurance)

Contractor Safety Procedures

Please reference and provide the following safety procedures to your contractor, subcontractors or any other relevant personnel. 

Occupational Safety Program Requirements

Contractors/subcontractors will:

  • Have required safety programs established for those projects they are completing at UNLV and will prepare/provide the necessary permits and forms to UNLV project managers.
  • Follow the safe practices prescribed for the jobs they will be doing and will have trained and certified operators in place (when required) for the equipment they will be using.
  • Provide the required personal protective equipment (PPE) for their own employees and ensure it is used.
  • Take the necessary steps to ensure that others in the area of the job site are not affected by the work taking place and any by-products produced.
  • Take the necessary steps to ensure that the proper housekeeping is being observed and the proper handling and storage of tools, materials and chemicals takes place. 
  • Have access to the UNLV Occupational Safety Programs and refer to them as needed throughout the project.
  • Immediately address safety concerns brought to their attention and discuss issues related to safe job performance with their project managers.
